FHA Services

In our family homes, individuals can interact with their friends and neighbors according to their own interests. They develop skills that are important to their lives and they receive the level of support that's right for them.  At California MENTOR we develop an Individualized Service Plan (ISP) to respond to the specific needs and preferences of each person we serve.  In addition, we continuously monitor those plans and make adjustments to meet each person’s changing needs.

Individualized Services and A Flexible Approach

Every service plan is different
but in general our FHA programs may include:

  • 24-hour home-based supervision
  • Collaborative assessments
  • Development & monitoring of ISP
  • Life skills development
  • Activities of daily living management
  • Structured activities
  • Advocacy services
  • Family Support
  • Community integration
  • Case management support
  • Medication management oversight
  • 24-hour on-call support
  • Relief Support
  • Program evaluation & quality assurance

Focused on Personal Choice.

Every individual we serve is actively involved in planning his or her daily routine and participating in family and community life.  By living in family homes individuals naturally build relationships, become part of the family and participate in everyday activities.  Through that participation and engagement in the family home individuals can also build skills and learn behaviors they can use in other community settings such as day programs and supported employment.
